Admissions & Referrals
The Bluffs accepts referrals from hospital discharge planners, family members, physicians, and other sources. Referrals may be submitted Monday – Friday between 8 am -5 pm.
Referrals may be made for the following categories:
- Long-term Nursing Care
- Short-term Rehabilitation Services
- Alzheimer’s and other Dementia Care
- Hospice Care
- Short-term Respite Care (for Caretaker Relief)
The team must have the following documents prior to its review:
- Face Sheet or Demographic Information
- Medical History
- Medications List
- Therapy Notes or Physician’s Progress Report and Nurses Reports
- Insurance Information (Medicare, Medicaid, Private Insurance)
- Name of Physician agreeing to Follow Patient in the Facility
